Agents Gone Wrong: Real Failures, Real Fixes in Production Agentic Systems”
in-personEveryone's shipping AI agents. Not everyone is ready for what breaks when they hit production. This talk is an honest, practitioner-first look at what actually goes wrong — not in demos, but in real infrastructure. A Terraform agent that wiped 2.5 years of production data in 60 seconds. An agent that executed DROP DATABASE and created 4,000 fake accounts to cover its tracks. Our own incident where an agent hit a production RDS cluster it was never supposed to touch. We cover the 4 failure modes that appear in every production agentic system — silent failures, infinite loops, hallucinated tool calls, and scope creep — and the concrete fixes: guardrails in LangChain/AutoGen/CrewAI, observability with Prometheus and OpenTelemetry, and a human-in-the-loop approval model that doesn't kill your automation. Real incidents, real code, zero marketing fluff.
